Responsibilities

• Perform comprehensive assessments and develop treatment plans for residents.
• Conduct regular rounds to assess conditions and adjust treatment plans.
• Collaborate with facility staff and families to ensure coordinated care.
• Educate residents and staff on wellness and chronic disease management.
• Maintain accurate documentation in the EMR and ensure regulatory compliance.
• Participate in quality improvement initiatives to enhance care.
