Qualifications

• Valid NP/PA license in state of operations and current board certification.
• Master's Degree in Nursing or PA Studies.
• Strong leadership, interpersonal, and problem-solving skills; excellent written and oral communication.

Responsibilities

• Perform comprehensive assessments and develop care plans for residents.
• Conduct routine rounds and adjust treatment plans as needed.
• Collaborate with facility staff and families to coordinate care.
• Educate residents and families on disease prevention and wellness.
• Maintain EMR documentation and ensure regulatory compliance.
• Participate in quality improvement to reduce readmissions and improve care.
