Job summary

Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care in senior living facilities. You will provide direct patient care, manage chronic conditions, and coordinate with other providers to promote wellness and independence for residents.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification in your field.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.
• Team-oriented with punctuality and organizational skills.

Responsibilities

• Perform comprehensive assessments, develop treatment plans, and provide ongoing care management for residents.
• Conduct regular rounds to assess residents’ conditions and adjust treatment plans.
• Collaborate with facility staff, family members, and other clinicians to coordinate care.
• Educate residents and families on disease prevention and wellness.
• Maintain accurate documentation in the EMR and ensure regulatory compliance.
• Participate in quality improvement to enhance care and reduce hospital readmissions.
