Curana Health seeks a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care in skilled nursing and senior living facilities. You will perform direct patient care to promote wellness, manage chronic conditions, and coordinate with other providers.

You will travel to multiple facilities, provide patient education, and contribute to care quality and outcomes in a supportive team environment.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification in your field.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills; punctuality and organization.

Responsibilities

• Provide direct patient care and assessments in SNF, assisted and independent living facilities.
• Perform routine rounds, adjust treatment plans, and manage chronic conditions.
• Coordinate care with facility staff, families, and other providers.
• Educate residents and families on wellness and disease prevention.
• Maintain accurate EMR documentation complying with regulatory standards.
• Participate in quality improvement initiatives to reduce readmissions and improve care.
