You will perform direct patient care, manage chronic conditions, and coordinate with other clinicians to improve health outcomes for seniors.

Qualifications

• Valid NP or PA license in operating state and current board certification.
• Master’s degree in nursing or PA studies.
• Strong leadership, interpersonal and problem-solving skills preferred.
• Excellent written and verbal communication skills.
• Punctuality, organization and teamwork required.

Responsibilities

• Patient care and assessment with comprehensive plans.
• Routine rounds and monitoring of residents.
• Care coordination with staff, family and providers.
• Patient education on wellness and chronic disease management.
• Accurate EMR documentation and regulatory compliance.
• Participate in quality improvement to reduce readmissions.
