Qualifications

• Valid NP or PA license in state of operations and current board certification.
• Master's Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.
• Punctuality and organizational skills; able to work within a team.

Responsibilities

• Provide direct patient care and manage chronic conditions for residents.
• Perform comprehensive assessments, develop treatment plans, and ongoing care management.
• Coordinate care with facility staff, families, and other healthcare providers.
• Educate residents, families, and staff on wellness and chronic disease management.
• Document accurately in the EMR and ensure regulatory compliance.
• Participate in quality improvement initiatives to enhance care and outcomes.
