Job summary

Optum HouseCalls is seeking an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members. The role emphasizes education, gap closure, and collaboration with primary care providers to improve outcomes.

Flex scheduling is offered across a 7-day week with no on-call, weekends, or holidays required. Competitive total compensation includes base pay and bonus with opportunities for growth within the Optum Home & Community Care organization.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• NCCPA certification and state licensure for PAs.
• Active, unrestricted licensure in the practicing state without disciplinary actions.
• Active and unrestricted driver's license and access to reliable transportation.
• Ability to navigate varied home environments to perform assessments.
• If state allows independent practice, obtain licensing board approval within required timelines.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage members and other plan members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to members’ PCPs to address gaps in care.
• Recognize urgent/emergent situations and intervene as needed.
• Educate members on diseases, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
