Job summary

Optum Home & Community Care seeks an Advanced Practice Clinician (NP or PA) to perform in-home preventive care, education, and coordination with primary care teams. This field-based role focuses on proactive health management with no on-call duties.

You will conduct assessments, document diagnoses, and connect patients to community resources while supporting health outcome improvements in Medicare Advantage and related programs.

Qualifications

• This is a non-prescribing, field-based role focused on preventive care.
• Advanced Practice Clinician (NP or PA) delivering home-based services.
• Collaborates with primary care providers to close care gaps.

Responsibilities

• Conduct comprehensive in-home health assessments including medical history and vitals.
• Document diagnoses to support care management and future planning.
• Coordinate with primary care providers to address gaps in care.
• Educate members on disease management, medications, and adherence.
• Identify urgent situations and take appropriate action.
• Assess social determinants of health and connect to resources.
