Job summary

Optum Home & Community Care is expanding its interdisciplinary in-home care model to help patients navigate the health system and access support services. This role emphasizes in-home assessments, evidence-based screenings, and care management with frequent travel to patient homes.

Active NP or PA licensure and national certification are required, with travel up to 85%. Compensation ranges from $109,500 to $164,000 with a comprehensive benefits package and incentive programs as part of Total

Qualifications

• Active NP or PA license or ability to obtain by start date; licensure must be unencumbered
• Active ANCC or AANP national certification in relevant family/gerontology/acute care paths or NCCPA for PAs
• Ability to lift 30 pounds, navigate stairs and fit assessment activities in various dwelling conditions
• Reliable transportation to complete home visit assessments

Responsibilities

• Conduct in-home assessments including medical history, review of systems, med reconciliation, vitals, and physical exam
• Perform screenings including point-of-care testing as appropriate
• Identify diagnoses for care management and active medical management
• Formulate and communicate findings to inform the member's PCP of gaps in care
• Identify urgent situations and intervene appropriately
• Educate members on disease processes, medications, and adherence
• Travel up to 85% to conduct visits
