Job summary

UnitedHealth Group, through Optum Home & Community Care, is expanding its in-home care team. The role involves conducting comprehensive in-home assessments, reviewing medical history, medications, and vital signs, and coordinating with primary care physicians to close gaps in care.

Qualifications

• Active NP or PA license, unencumbered and in good standing.
• National certification in Family, Geriatrics, Adult, or related field or ability to obtain by start date.
• Ability to lift 30 pounds, navigate stairs, and perform physical assessments.
• Reliable transportation to complete home visit assessments.

Responsibilities

• Conduct in-home assessments: history, systems, med review, vitals, exam.
• Perform screenings and point-of-care testing as appropriate.
• Identify diagnoses for care management and active medical management.
• Summarize findings to inform the member's PCP of gaps in care.
• Identify urgent situations and intervene as needed.
• Educate members on disease, medications, and adherence.
• Travel up to 85% to visit patients.
