Job summary

Optum Home & Community Care is hiring an Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage and other plans. This field-based, non-prescribing role focuses on education, gap closure, and collaboration with primary care providers.

Flexible scheduling with 20-40 hours per week available, no on-call, weekends or holidays required, and additional compensation for multilingual candidates. Join a team delivering care at patients' homes.

Qualifications

• Active ANCC or AANP certification for NP
• Current NCCPA certification and state licensure for PA
• Active, unrestricted licensure in practicing state
• Driver's license and reliable transportation for home visits
• Ability to navigate varied home environments and assist admissions

Responsibilities

• Conduct comprehensive in-home assessments including history, vitals and exams
• Identify and document diagnoses for care management and planning
• Communicate findings to PCPs to close gaps in care
• Recognize urgent situations and intervene appropriately
• Educate members on diseases, medications and adherence
• Address social determinants of health and provide referrals as needed
