Qualifications

• Active NP or PA certification (ANCC/AANP for NP, NCCPA for PA).
• Active, unrestricted state licensure and ability to obtain start date.
• Driver's license with reliable transportation for home visits.
• Eligibility to practice independently where allowed; license approval if required.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Review past medical history, reconcile medications, take vital signs, and perform exams.
• Conduct in-home health assessments and screenings.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care.
• Educate members on diseases, medications, and adherence.
• Identify urgent situations and intervene appropriately.
• Address social determinants of health and provide referrals as needed.
