Job summary

Qualifications

• Active NP or PA with 4+ years delivering direct clinical care.
• Oncology, home care, palliative, hospice or primary care experience required.
• Proficient with Slack, Zoom, Google Suite, and EHR systems.
• Oncology Nursing Society certification a plus.

Responsibilities

• Provide direct patient care in home and via virtual platforms.
• Triage high-acuity patients and coordinate with oncology practices.
• Support patients across the full care continuum.
• Build trusting relationships with patients and care networks.
• Collaborate to refine and implement the care model.
