Job summary

Daymark Health Inc. seeks a skilled Nurse Practitioner or Physician Assistant to join our home-based oncology care team in San Diego.

You will deliver direct patient care in homes and through virtual visits, triage high-acuity cases, and collaborate with the clinical operations team to scale our evidence-based oncology care model. Qualifications

• Active NP or PA with at least 4 years of direct clinical care.
• Oncology, home care, palliative care, hospice, or primary care experience.
• Certification in Oncology Nursing Society is a plus.

Responsibilities

• Provide direct patient care in patients' homes and via virtual platforms.
• Triage high-acuity patients and coordinate with oncology practices.
• Support patients along the full care continuum from active treatment to survivorship/end-of-life care.
• Build trusted relationships with patients and care networks through empathy and active listening.
• Collaborate with operations to refine and implement Daymark's care model for the San Diego market.
