Job summary

Daymark Health seeks an experienced Nurse Practitioner or Physician Assistant to join our home-based oncology care team. You will deliver hands-on clinical care in patients' homes and through virtual visits, supporting a high-performing clinical operation.

You’ll work as a player/coach, triaging high-acuity patients, collaborating with oncology practices, and helping to implement our evidence-based care model in the Inland Empire region, with a focus on compassionate, coordinated care.

Qualifications

• Active NP or PA with at least 4 years of direct patient care.
• Experience in oncology, home care, palliative or hospice a plus.
• Oncology Nursing Society certification and value-based care experience are a plus.

Responsibilities

• Provide direct clinical care in patients' homes and via virtual platforms.
• Place orders, prescribe medications, document appropriately, and coordinate care.
• Triage high-acuity patients and collaborate with oncology practices.
• Support patients along the full care continuum from active treatment to survivorship.
• Build trusting relationships with patients and care networks.
• Help refine and implement the Daymark care model.
