Qualifications
- Minimum of 6 months Nurse Practitioner experience in primary care/internal medicine.
- Value-based care experience preferred.
- Basic Life Support (BLS) certification is required.
- Must be comfortable providing care in patients' homes.
Responsibilities
- Develop and implement clinical plans of care for adult patients with chronic conditions.
- Gather patient history, physical exams, and diagnostics in home or clinic settings.
- Collaborate with the interdisciplinary team to meet patients' needs.
- Educate patients about medication usage and illness progression.
