AlignMed Partners is seeking a Full Time Advanced Practice Provider (APN/PA) to deliver primary and urgent care at the Grand Islander Center in Middletown, RI. The role supports residents of skilled nursing facilities and works with a leadership team to achieve high-quality outcomes.

Qualified candidates include NPs with geriatric or family focus and PAs from ARC-PA programs, with RN licensure and NCCPA/PA licensure in good standing. Two years' geriatric experience is preferred.

Qualifications

• The Advanced Practice Provider must be a graduate from an accredited School of Nursing with RN licensure and NP certification (Master's).
• PA must be graduate from ARC-PA accredited program and pass PANCE with PA licensure and NCCPA certification.
• Minimum two years' experience with adult/geriatric populations preferred.

Responsabilités

• Provide primary and urgent care services to nursing center residents.
• Collaborate on-site with Center Leadership to meet goals and priorities.
• Coordinate with Medical Director/Attending Physician for patient care.
