Job summary

AlignMed Partners in Epsom, NH is seeking a full-time Advanced Practice Provider (NP or PA) to deliver primary and urgent care to long-term care residents. New grads welcome; flexible schedule; no on-call or weekend shifts.

The base salary ranges from $116,480 to $128,960 annually, plus a no-cap incentive program and yearly bonus opportunities. This role reports to Center Leadership and collaborates with the Medical Director to provide high-quality care across the center, with a focus on

Qualifications

• Graduate from an accredited nursing program with RN licensure.
• NP or PA with appropriate credentialing and state licensure.
• PANCE and NCCPA certification where applicable.
• Two years of adult or geriatric care experience preferred.
• DEA licensed where required and related state requirements.

Responsibilities

• Provide primary and urgent care to nursing residents in collaboration with the center team.
• Collaborate with Medical Director and Center Leadership for care plans.
• Support high-quality outcomes and compassionate care delivery.
