Preferred experience includes geriatrics, post-acute care, or internal medicine, with opportunities to expand clinical expertise. Strong emphasis on evidence-based practice and patient education.

Qualifikationen

• Active state license for CA as NP or PA.
• National certification appropriate to the profession.
• Experience in post-acute care, SNF/ALF preferred.
• Knowledge of internal medicine, geriatrics, primary care management.
• Familiarity with EMR, CPT coding, ICD-10 coding.
• Strong communication and collaboration skills.

Aufgaben

• Perform comprehensive patient evaluations and med reviews.
• Diagnose and manage acute and chronic conditions.
• Review labs/imaging to guide treatment decisions.
• Manage medications and reconcile polypharmacy.
• Prescribe within scope of practice and manage orders.
• Coordinate care with physicians and nursing staff.
• Educate patients and families on diagnoses and medications.
