Job summary

TruHealth in Grand Island, NE is seeking an advanced practice provider (NP/PA) with primary care or internal medicine experience, preferably geriatrics, to join a team caring for long-term care residents and coordinating with case managers.

You will exercise independent judgement with supervision, prescribe meds, order labs, and work with facility partners to implement our model of care in compliance with regulations.

Qualifications

• Masters degree or higher in nursing or physician assistant studies.
• APRN or PA license from a recognized program.
• Minimum 3 years of clinical nursing or rehab experience in geriatric populations.
• Experience with Electronic Health Records and basic Microsoft Office tools.

Responsibilities

• Assess and plan care for patients, coordinating with an integrated care team.
• Prescribe medications and order labs, imaging, and consultations as needed.
• Ensure compliance with local, state, and federal regulations governing clinical services.
• Monitor patient response to treatment and adjust plans accordingly.
• Collaborate with RN case managers and facility staff to support care goals.
