HealthDrive is seeking a Nurse Practitioner to manage medical needs for residents in sub-acute rehab and long-term care facilities. You will diagnose, treat, and coordinate care while collaborating with administrators and staff to ensure high-quality on-site medical services.

The role emphasizes independent practice with a strong emphasis on geriatric health, preventive care, and patient education, supported by a 3-week training program and robust clinical mentorship.

Qualifications

• Experience in geriatric or Medicare populations and strong clinical assessment skills.
• Able to conduct physical examinations, interpret labs, and manage post-acute care.
• Licensed Nurse Practitioner with board certification and DEA/CS credentials.

Responsabilités

• Provide primary, post-acute, and preventative care for residents in sub-acute and long-term care units.
• Order and interpret diagnostic tests and imaging.
• Diagnose conditions and propose treatments for chronic and infectious diseases.
• Prescribe medications and manage treatment plans.
• Maintain accurate medical records and care schedules.
• Educate patients and families about health conditions and medications.
• Oversee overall patient health and coordinate with on-site staff.
