Job summary

HealthDrive is seeking a part-time Nurse Practitioner (3 days/week) to manage medical needs in sub-acute rehab and long-term care facilities. You will diagnose, treat, and promote health while collaborating with the medical team and building relationships with facility staff.

The role includes a 3-week paid training program with mentorship, comprehensive benefits for eligible positions, and opportunities for professional growth in post-acute care settings.

Qualifications

• Master’s degree from an accredited program.
• Active State Nurse Practitioner license required.
• Board-certified NP in relevant specialty(es).
• DEA/Controlled Substance license required.
• Valid driver’s license is a must.
• Ability to work independently with on-site staff.

Responsibilities

• Provide primary, post-acute, and preventive care for residents in sub-acute rehab and long-term care.
• Order and study diagnostic tests (blood tests, X-rays).
• Diagnose health conditions and illnesses.
• Propose treatments for chronic and infectious diseases.
• Prescribe medications.
• Maintain accurate records and schedules.
• Inform patients about health conditions and prescribed medications.
• Oversee patients’ overall health.
