Job summary

Provider Partners Health Plan is seeking an Institutional SNP Nurse Practitioner to deliver coordinated care in designated skilled nursing facilities. You will work with the PCP to implement preventive and acute care plans, interpret tests, and prescribe medications as appropriate.

You will collaborate with the care team, document to PPHP standards, and support families and staff with clear communication and care transitions.

Qualifications

• Licensed NP with current state registration and privileges.
• Experience in SNF/LTC preferred; case management experience a plus.
• Knowledge of I-SNP MOC and Medicare/Medicaid programs.
• Proficient in medical databases and MS Office.

Responsibilities

• Conduct comprehensive health assessments with autonomy.
• Coordinate care plans with PCP and interdisciplinary team.
• Document per PPHP standards and ensure accountability.
• Adhere to scope of practice in the licensed state.
• Communicate transitions of care with physicians and families.
• Provide preventive care and timely interventions.
