Job summary
Provider Partners Health Plan is seeking an ISNP Nurse Practitioner to deliver coordinated care at designated skilled nursing facilities, collaborating with the member's PCP to provide preventive and acute/chronic care. This role acts as the liaison to the interdisciplinary team, coordinating tests, treatments, and care plans. The NP prescribes medications and guides short- and long-term goals for members, families, and staff.
Qualifications
Minimum of 3 years clinical care experience.
Skilled Nursing Facility/Long Term Care experience preferred.
Knowledge of Medicare/Medicaid and I-SNP MOC.
Responsibilities
Demonstrates autonomy in performing comprehensive health assessments.
Collaborates with PCP to create preventive care plans.
Adheres to PPHP documentation standards and communication.
Observes scope of practice as licensed in the state.
Communicates with physicians and family during transitions of care.
