Job summary

UnitedHealth Group’s Optum HouseCalls team seeks an Advanced Practice Clinician (NP or PA) to perform comprehensive in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve health outcomes.

Flexible scheduling is offered with no on-call, weekends, or holidays required. Active licensure and national certifications are required, with a compensation package including base pay and potential bonuses.

Qualifications

• Active clinical certifications (ANCC/AANP) required.
• NCCPA certification for PAs and state licensure required.
• Active, unrestricted licensure in practicing state; driver’s license and reliable transportation needed.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to close gaps in care and coordinate referrals.
