UnitedHealth Group's Optum HouseCalls team is seeking an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members, focusing on education, gap closure, and collaboration with primary care providers.

Qualifications
- Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
- Current NCCPA certification for PAs and state licensure
- Active and unrestricted licensure in the practicing state
- Driver's license and reliable transportation for home visits
Responsibilities
- Conduct comprehensive in-home health assessments.
- Coordinate with PCPs to close care gaps.
- Educate patients on diseases, medications and adherence.
- Identify urgent situations and intervene appropriately.
- Address social determinants of health and arrange referrals.
