Job summary

Optum Home & Community Care, part of UnitedHealth Group, is hiring HouseCalls Advanced Practice Clinicians (NP/PA) to conduct in-home health assessments for Medicare Advantage members across Massachusetts.

Role requires bilingual Spanish speakers, extensive travel (~85%), travel expenses covered, and flexible scheduling (20–40 hours/week). No on-call, weekends, or holidays. Competitive base pay with incentives and comprehensive benefits.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in the practicing state with start-date eligibility.
• Driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.
• Bilingual in Spanish.
• Eligibility/approval steps may be required within specified timelines.

Responsibilities

• Conduct in-home assessments for Medicare Advantage and other plan members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care.
• Recognize urgent/emergent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
