Job summary

Nephrology Associates Medical Group is seeking a Nurse Practitioner to provide comprehensive patient care for CKD and ESRD across outpatient, hospital, and dialysis settings. You will assess, diagnose, and treat patients within licensure and practice guidelines, collaborating with physicians and the care team.

This role emphasizes education, care coordination, and long-term disease management, with on-call coverage and dialysis rounding.

Qualifications

• Master's degree in NP program and active NP license.
• Prescriptive authority as per state requirements.
• Experience in nephrology, internal medicine, hospital medicine, dialysis or related setting preferred.

Responsibilities

• Provide direct patient care for CKD/ESRD patients in outpatient, hospital, and dialysis settings.
• Develop and update treatment plans based on clinical findings.
• Diagnose and manage acute, chronic, and complex conditions in collaboration with physicians.
• Perform and interpret diagnostic tests and order treatments as appropriate.
• Conduct CKD clinic visits, hospital rounds, dialysis rounding, and on-call coverage.
• Educate patients and families to promote health and disease management.
