Job summary

U.S. Renal Care, Inc. is seeking a licensed Advanced Practitioner to deliver comprehensive ESKD care, including rounding and patient education, under nephrologist guidance.

The role emphasizes holistic illness management, coordination with dialysis teams and hospital discharge teams, and strong communication with patients and families. The candidate should have a state Advanced Practitioner license, nephrology experience, and excellent written and verbal communication skills.

Qualifications

• Current license to practice as an Advanced Practitioner in the state
• Firm understanding of the protocols and operations of a physician practice
• Specialty certification and 2 years clinical nephrology experience preferred
• Excellent written and verbal communication skills

Responsibilities

• ESKD rounding, including limited and comprehensive visits under nephrologists guidance.
• Holistic ESKD patient care management including activation, depression, hospitalization risk, transition of care, and comorbidity oversight.
• ESKD patient education and annual comprehensive disease management visits.
• Collaborate with nephrologists, dialysis center teams, hospital discharge teams, PCPs and sub-specialists as clinically indicated.
• CKD clinical care and kidney disease education.
• Conduct physical exams, order and assess diagnostics, plan and initiate treatment, and prescribe medications.
• Document medical information in EMR and educate patients and families on health problems and home care.
• Promote GUEST customer service standards and teamwork; adhere to company policies.
• Maintain regular attendance and reliability.
