Job summary

U.S. Renal Care, Inc. is seeking an Advanced Practitioner to provide specialized care for patients with end-stage kidney disease (ESKD) in a nephrology setting. The role includes rounding with nephrologists, holistic disease management, education, and annual care planning.

You will collaborate with dialysis teams, hospital discharge coordinators, PCPs, and subspecialists to optimize patient outcomes. Strong communication and clinical certification are required for success.

Qualifications

• Current license to practice as an Advanced Practitioner in the state performing duties
• Firm understanding of protocols, policies and operations of a physician practice
• Must have necessary specialty certification; 2 years of clinical work experience; Nephrology experience a strong plus
• Excellent written and verbal communication skills

Responsibilities

• ESKD rounding, including limited and comprehensive visits under guidance of practice nephrologists
• ESKD holistic patient care management including activation, depression, high risk of hospitalization, transition of care and comorbidity oversight
• ESKD patient education
• ESKD comprehensive disease management annual visits
• Care collaboration with nephrologists, dialysis center IDT, Kidneylink IDT, hospital discharge care coordination team, PCPs and sub-specialists
• CKD clinical care and kidney disease education
• Other duties and tasks as assigned
• Perform physical examinations of patients to evaluate health and diagnose illnesses
• Order and assess diagnostic tests such as X-rays and laboratory tests
• Determine, plan, and initiate appropriate treatment, and adjust treatment if necessary
• Prescribe medications
• Document medical information and maintain EMR
• Educate, instruct, and counsel patients and families on health problems and home care
• Request consultation with other providers and collaborate with Kidneylink RN Care Manager
• Promote GUEST customer service standards and build relationships
• Participate in team concepts and adhere to company policies
• Maintain regular and reliable attendance
