Job summary

MDB Health Services seeks an experienced Nephrology Nurse Practitioner to join the clinical team and provide specialized evaluation, management, and ongoing renal care for patients with kidney disease. The role collaborates with nephrologists, dialysis teams, nursing staff, and facility staff to deliver high-quality care in long-term care settings.

Qualifications

Current, unrestricted NP/APRN license in the state of practice.
2+ years of Nurse Practitioner experience in Nephrology preferred.
Strong clinical assessment and diagnostic skills.
Experience in a long-term care, skilled nursing, dialysis, hospital, or outpatient nephrology setting.

Responsibilities

Evaluate, diagnose, and manage patients with acute and chronic kidney disease.
Provide ongoing management of CKD, ESRD, and renal-related complications.
Perform comprehensive nephrology-focused assessments and develop individualized treatment plans.
Monitor and manage progression of chronic kidney disease.
Review renal-related laboratory and diagnostic results.
Identify and manage complications associated with CKD and ESRD.
Monitor and manage fluid status, edema, electrolyte abnormalities, and blood pressure in renal disease.
Review medication regimens for renal dosing and nephrotoxic meds.
Assist with evaluation and management of renal hypertension and related conditions.
Participate in interdisciplinary nephrology case planning and discussions.
Communicate significant changes to supervising nephrologist and healthcare team.
