Job summary

Page Mechanical Group, Inc. is seeking a Nurse Practitioner to provide comprehensive care for patients with chronic kidney disease, end-stage renal disease, and related conditions across outpatient clinics, hospitals, and dialysis settings.

You will collaborate with physicians and the healthcare team to assess, diagnose, and manage complex patient cases. The role emphasizes clinical judgment, patient education, and coordination of care, with on-call coverage and involvement in CKD/ESRD hospital

Qualifications

• Master's degree from an accredited NP program.
• Current NP license and RN license, as applicable by state.
• Board certification as NP preferred depending on employer requirements.
• Prescriptive authority required, per state and position requirements.
• Experience in nephrology, internal medicine, hospital medicine, dialysis, chronic disease management, or related clinical settings preferred.
• Experience managing CKD/ESRD patient care preferred.

Responsibilities

• Provide direct patient care for CKD/ESRD across outpatient clinic, hospital, and dialysis settings.
• Conduct patient assessments and review medical histories, symptoms, and diagnostic results to determine diagnoses and treatment needs.
• Develop, implement, and update treatment plans based on clinical findings and guidelines.
• Diagnose, treat, and manage acute, episodic, chronic, or complex conditions in collaboration with physicians.
• Manage long-term care plans for adults with chronic conditions.
• Order, perform, and interpret diagnostic tests; prescribe medications as appropriate.
• Conduct CKD clinic visits and CKD/ESRD hospital visits; coordinate with hospital providers.
• Perform dialysis rounding and assist with care planning; participate in on-call coverage.
