Job summary

Duo Health in Casa Grande, AZ seeks a Nurse Practitioner to deliver CKD and complex chronic disease care in patients' homes, via telehealth, and through coordinated care with a multidisciplinary team.

You will educate patients, coordinate with physicians, social workers, and care navigators, and practice in a value-based, relationship-centered model to improve outcomes and reduce hospitalizations.

Qualifications

• Active NP/PA license (AZ or FL) + board certification.
• Background check required.
• Reliable transportation and ability to travel to patient homes.

Responsibilities

• Manage a small panel of CKD and other complex chronic conditions with an interdisciplinary team.
• Conduct health assessments, histories, physicals, and ongoing evaluations.
• Diagnose, treat, and manage acute and chronic conditions within your scope.
• Order, interpret, and review labs and diagnostic testing.
• Prescribe and manage medications per state regulations.
• Develop individualized care plans addressing medical, behavioral, and social needs.
