Job summary

Duo Health is seeking a Nurse Practitioner to join a care team providing high-quality value-based care for CKD patients. The NP will travel to patient homes in the Miami area, interact with patients via home visits, teleconference, or telephone, and coordinate with a multidisciplinary team to ensure optimal patient outcomes.

The role operates Monday–Friday, 8:00 am–5:00 pm, with duties spanning assessment, education, and care planning in patients’ homes and community settings.

Qualifications

• Licensed Advanced Practice Nurse (APN) in Florida.
• Must meet Duo Health clinician credentialing criteria, including background check.
• Reliable transportation and verifiable good driving record; must travel to patient homes.
• Ability to develop inclusive and equitable relationships with patients, caregivers and staff.
• Experience working with diverse community resources.

Responsibilities

• Provide medical care for CKD patients in collaboration with the care team.
• Perform history and physical exams; order labs and tests as needed.
• Educate patients about medications and side effects; prescribe per state authority.
• Coordinate care across settings and with care team members.
• Be available for after-hours telephonic support as needed.
• Document encounters timely and accurately; maintain confidentiality.
