Job summary

Duo Health in Tampa, FL seeks a Licensed Advanced Practice Nurse to join our in-home care team for patients with chronic kidney disease. The role involves direct patient assessment, telehealth coordination, and collaboration with physicians, nurses, and social workers in a high-touch care model.

You will travel to patient residences across the Tampa area, contribute to comprehensive care plans, and participate in ongoing quality improvement initiatives within a mission-driven organization.

Qualifications

• Licensed Advanced Practice Nurse (APN) in Florida.
• Excellent communication skills, written and oral.
• Ability to travel to patient homes and work across Tampa area.

Responsibilities

• Provide clinical services for CKD patients in home or telehealth settings.
• Coordinate care with the multidisciplinary team.
• Document all patient encounters accurately and timely.
• Develop and implement care plans and participate in team huddles.
• Maintain confidentiality and adhere to policies.
