Job summary

Duo Health in Navajo County, AZ seeks a Nurse Practitioner to join an interdisciplinary care team delivering CKD and complex chronic disease management in patients' homes, via telehealth, or by phone. You’ll focus on preventive care, patient education, and care coordination within a value-based, relationship-centered model.

Qualifications

• Active NP/PA license (AZ or FL) + board certification.
• Must meet clinician credentialing requirements, including a background check.

Responsibilities

• Manage a small panel of patients with CKD and other complex chronic conditions alongside an interdisciplinary care team.
• Conduct comprehensive health assessments, histories, physical exams, and ongoing clinical evaluations.
• Diagnose, treat, and manage acute and chronic conditions within your scope of practice.
• Order, interpret, and review labs and diagnostic testing.
• Prescribe and manage medications in accordance with state regulations.
• Build individualized care plans addressing each patient’s medical, behavioral, and social needs.
• Educate patients and caregivers on disease prevention, medication adherence, lifestyle changes, and treatment plans.
• Coordinate care across primary care providers, specialists, hospitals, home health, hospice, and community organizations.
• Participate in regular interdisciplinary care team huddles.
• Monitor patient progress and adjust care plans to improve outcomes and reduce avoidable hospitalizations.
• Document clinical encounters accurately and on time.
• Support quality improvement initiatives and evidence-based practice.
• Maintain patient confidentiality and comply with company policy and regulatory requirements.
• Provide after-hours telephonic support as needed
