Job summary

Duo Health is seeking a Nurse Practitioner to join an interdisciplinary team providing comprehensive medical care for CKD patients and other complex conditions. You will focus on preventive care, disease management, patient education, and care coordination within a value-based model.

You will work with physicians, social workers, care navigators, and community resources, delivering care in patients' homes, virtually, or by phone to build strong, long-term relationships and improve outcomes.

Qualifications

• Active NP/PA license (AZ or FL) + board certification.
• Meet Duo Health's clinician credentialing requirements, including a background check.
• Reliable transportation, a valid driver's license, and the ability to travel to patient homes.
• Strong clinical assessment, communication, and critical-thinking skills.
• Comfort working independently while collaborating within a team.
• Comfort delivering care both in person and via telehealth.
• A genuine passion for relationship-centered, value-based care for patients with chronic illness.

Responsibilities

• Manage a small panel of patients with CKD and other complex chronic conditions alongside an interdisciplinary care team.
• Conduct comprehensive health assessments, histories, physical exams, and ongoing clinical evaluations.
• Diagnose, treat, and manage acute and chronic conditions within your scope of practice.
• Order, interpret, and review labs and diagnostic testing.
• Prescribe and manage medications in accordance with state regulations.
• Build individualized care plans addressing each patient's medical, behavioral, and social needs.
• Educate patients and caregivers on disease prevention, medication adherence, lifestyle changes, and treatment plans.
• Coordinate care across primary care providers, specialists, hospitals, skilled nursing facilities, home health, hospice, and community organizations.
• Participate in regular interdisciplinary care team huddles.
• Monitor patient progress and adjust care plans to improve outcomes and reduce avoidable hospitalizations.
• Document clinical encounters accurately and on time.
• Support quality improvement initiatives and evidence-based practice.
• Maintain patient confidentiality and comply with company policy and regulatory requirements.
• Provide after-hours telephonic support as needed.
