Job summary

Duo Health in Apache Junction, AZ is seeking a Nurse Practitioner to join an interdisciplinary team delivering preventive care and chronic disease management for CKD patients. Care is provided in patients' homes, via telehealth, or by phone, emphasizing relationship-based, value-driven care.

You will collaborate with physicians, social workers, care navigators, and community partners to offer comprehensive, high-touch care and ensure care is coordinated around the whole person.

Qualifications

• Active NP license (AZ or FL) + board certification
• Ability to meet credentialing requirements, including background check
• Reliable transportation and travel to patient homes
• Strong clinical assessment and communication skills; independent yet team-oriented
• Comfort delivering care in person and via telehealth

Responsibilities

• Manage a small panel of CKD patients with an interdisciplinary team
• Conduct comprehensive health assessments, histories, and physicals
• Diagnose, treat, and manage acute and chronic conditions
• Order and interpret labs and diagnostic testing
• Prescribe and manage medications within state scope
• Build individualized care plans addressing medical, behavioral, and social needs
• Educate patients and caregivers on prevention, adherence, and lifestyle changes
• Coordinate care across primary care, specialists, and community resources
• Participate in interdisciplinary care team huddles
• Monitor progress and adjust plans to reduce hospitalizations
• Document encounters accurately and timely
• Support quality improvement and evidence-based practice
