Duo Health in Tolleson, AZ is hiring a Nurse Practitioner to join an interdisciplinary care team delivering CKD-focused, value-based care in patients' homes, via telehealth, or by phone.

You will conduct comprehensive assessments, manage chronic conditions, educate patients, and coordinate care with physicians, social workers, and community resources.

The role emphasizes relationship-centered care, collaboration, and flexible scheduling to support high-quality outcomes.

Qualifications

• Active NP/PA license (AZ or FL) and board certification.
• Ability to meet credentialing requirements, including background check.
• Reliable transportation and a valid driver's license.
• Strong clinical assessment, communication and critical-thinking skills.
• Comfort delivering care in person and via telehealth.

Responsabilités

• Manage a small panel of CKD patients and coordinate with an interdisciplinary team.
• Conduct comprehensive health assessments, histories, physical exams, and ongoing evaluations.
• Diagnose, treat, and manage acute and chronic conditions within scope.
• Order, interpret, and review labs and diagnostic tests.
• Prescribe and manage medications per state regulations.
• Develop individualized care plans addressing medical, behavioral, and social needs.
