Job summary

Duo Health in Mesa, AZ is hiring a Nurse Practitioner to manage a focused CKD patient panel within an interdisciplinary team. You will deliver preventive and chronic care, coordinate with physicians, social workers, and care navigators, and provide education to patients and families in home, telehealth, or phone visits.

You will work with physicians, care navigators, and community resources to ensure coordinated, high‑quality care centered on the whole person, while maintaining up‑to‑date

Qualifications

• Ability to meet Duo Health's clinician credentialing requirements, including a background check.
• Willingness to travel to patient homes.
• Experience delivering care in home-based or value-based care settings.

Responsibilities

• Manage a small panel of CKD and other complex chronic patients with an interdisciplinary team.
• Conduct comprehensive health assessments, histories, physical exams, and ongoing evaluations.
• Diagnose, treat, and manage acute and chronic conditions within scope of practice.
• Order, interpret, and review labs and diagnostic testing.
• Prescribe and manage medications per state regulations.
• Build individualized care plans addressing medical, behavioral, and social needs.
• Educate patients and caregivers on disease prevention, adherence, and lifestyle changes.
• Coordinate care across primary care, specialists, hospitals, and community resources.
• Participate in regular interdisciplinary care team huddles.
• Document encounters accurately and on time; support quality improvement initiatives.
• Provide after-hours telephonic support as needed.
