Curana Health is seeking Nurse Practitioners and Physician Assistants to provide proactive, relationship-based primary care to seniors in our care communities. You will deliver high-quality care directly at the residents' homes and facilities, supported by an experienced team and advanced technology.

Qualifications

• Valid NP/PA license in state of operations and current board certification.
• Masters degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal and problem-solving skills preferred.
• Excellent written and oral communication skills.
• Punctual, organized, and able to function in a team.

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing, assisted living, and independent living communities.
• Perform comprehensive health assessments and develop individualized treatment plans.
• Diagnose, treat, and manage acute and chronic conditions with preventative focus.
• Build meaningful patient relationships and conduct regular facility rounds.
• Collaborate with physicians, staff, facility leadership, specialists, and families.
• Maintain accurate EMR documentation and ensure regulatory compliance.
