Curana Health is seeking Nurse Practitioners and Physician Assistants to provide proactive primary care in senior living communities, partnering with physicians and care teams to manage residents across skilled nursing and assisted living settings.

The role emphasizes autonomous practice, travel between facilities, strong relationships with residents and families, and participation in a technology-enabled care model with a comprehensive benefits package.

Qualifications

• Valid NP or PA license in the state of operations
• Current board certification in your field
• Master's Degree in Nursing or PA Studies

Responsibilities

• Deliver high-quality primary care and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management to improve quality of life while helping residents stay healthier and more independent.
• Collaborate with physicians, clinical staff, and facility leadership to coordinate comprehensive care plans.
• Maintain accurate documentation in the EMR and ensure compliance with regulatory standards.
