Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care in skilled nursing facilities and senior living communities. You will provide direct patient care focused on wellness, chronic disease management, and coordinated care with other healthcare providers.

The role emphasizes autonomous practice with strong clinical and administrative support, opportunities to grow, and a schedule that includes travel to multiple facilities.

Qualifications

• Must hold a valid nurse practitioner or physician assistant license in your state of operation
• Current board certification in your field
• Masters degree in nursing or PA studies; strong leadership and communication skills preferred
• Excellent written and oral communication skills; punctuality and organizational skills expected

Responsibilities

• Provide direct patient care and manage chronic conditions in SNF and senior living facilities
• Perform comprehensive assessments and develop treatment plans
• Conduct routine rounds and adjust care as needed
• Coordinate care with facility staff, families and providers
• Document thoroughly in EMR and ensure regulatory compliance
• Engage in quality improvement to enhance care and reduce readmissions
