Qualifications

• Current license as a nurse practitioner or physician assistant in the practicing state.
• Master's degree in nursing or physician assistant studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Deliver high-quality primary care to residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with focus on prevention and long-term wellness.
• Provide ongoing care management to improve quality of life and independence.
• Conduct facility rounds and build strong relationships with residents, families, and care teams.
• Collaborate with physicians, clinical staff, and leadership to coordinate comprehensive care plans.
• Document accurately in the EMR and ensure regulatory compliance.
• Support initiatives to improve patient outcomes and reduce avoidable hospital visits.
