Curana Health is seeking Nurse Practitioners and Physician Assistants to deliver proactive primary care to seniors in our affiliated communities. You will practice at the top of your license, with autonomy and support from experienced physicians, operational leaders, and advanced technology.

The role emphasizes relationship-based care, flexible scheduling, and a mission-driven culture. You will collaborate across care teams to improve outcomes while providing compassionate, personalized services.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations.
• Current board certification in your field.
• Masters degree in nursing or physician assistant studies.

Responsibilities

• Perform comprehensive health assessments and develop individualized treatment plans.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventive care.
• Provide ongoing care management to improve quality of life and independence.
• Build meaningful patient relationships through regular rounds and collaboration with families and care teams.
• Monitor health status and intervene to prevent complications and hospitalizations.
• Educate residents and caregivers on disease management and wellness strategies.
• Coordinate care with physicians, clinical staff, facility leadership, specialists, and family members.
• Maintain accurate EMR documentation and ensure compliance with standards.
