Job summary

Circadia Health seeks a Nurse Practitioner to deliver direct patient care across skilled nursing facilities and home health programs. You will act on real-time AI signals, extend high‑quality care to patients at home, and train facility teams to integrate Circadia into clinical workflows.

Requires CA NP license with ANCC certification, 2+ years of experience with complex adult populations, strong independent practice skills, and willingness to travel locally within a 30–40 mile radius; Spanish

Responsibilities

Direct patient care: perform initial and follow-up assessments and develop plans of care.
Tech to bedside integration: train staff and embed Circadia in workflows.
Data-to-action: interpret trends and prioritize high-risk patients for interventions.
Documentation: complete notes within 24 hours of visits.
High-volume rounding: manage 25+ visits per day across SNFs/home visits.
Mentor: support NP onboarding and collaborate with ops/product teams.
