Job summary

Circadia Health in Detroit is seeking a hands-on Nurse Practitioner to lead bedside care within our AI-enabled platform. You will perform initial and follow-up assessments, coordinate with facility staff, and integrate Circadia analytics into clinical workflows through training and live demonstrations.

The role emphasizes independent practice, mobility within a 30-40 mile radius, and collaboration with clinical ops/product teams to drive proactive care and reduce readmissions.

Qualifications

• Certified and licensed Nurse Practitioner in MI; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid MI driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required

Responsibilities

• Direct patient care: conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech to bedside integration: train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
• Close the loop: complete documentation/charts within the same day.
• High-volume rounding: 30+ visits/day across SNFs; focus on new admits and Circadia alerts.
• Quality & product feedback: surface field insights to improve workflows and tech integration.
• Mentor & collaborate: support NP onboarding and partner with clinical ops/product teams.
• Regional flexibility: cover sites within a 30-40‑mile radius; mileage and lodging covered per policy.
