Job summary

Circadia Health seeks a Nurse Practitioner to lead patient care across skilled nursing facilities and Home Health AI Care programs. You will assess patients, act on predictive health signals, and coach facility teams to implement earlier interventions that reduce readmissions.

Based in NY with regional travel, you will blend direct care with tech-to-bedside integration, hands-on training, live demos, and collaborative program development.

Qualifications

• Licensed Nurse Practitioner in NY with ANCC or equivalent certification.
• 2+ years NP experience with complex adult populations.
• Eligible for Medicare participation; company bills on your behalf.
• Able to travel locally within NY; valid NY driver's license.

Responsibilities

• Direct patient care in SNF and Home Health AI programs.
• Train staff to integrate Circadia into clinical workflows.
• Interpret AI-driven insights to prioritize high-risk patients."
• Complete documentation within same day.
• Cover SNF sites and patient homes within a 30–40 mile radius.
