Job summary

Circadia Health is seeking a Nurse Practitioner to join our on-site team in New York. You will see patients first, act on real-time predictive signals, and coach facility staff to turn data into earlier interventions that reduce readmissions.

You'll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments, while mentoring onboarding and partnering with clinical ops and product teams.

Qualifications

• Must be a certified NP licensed in NY.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid NY driver's license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required.

Responsibilities

• Direct patient assessments and follow-ups on flagged residents.
• Train and coach staff at the point of care; standardize Circadia usage.
• Interpret trend-based insights; prioritize high-risk patients; drive timely interventions.
• Complete notes and charts within the same day.
• Perform high-volume rounds across SNFs (30+ visits/day where appropriate).
• Surface feedback to improve workflows and tech integration.
• Mentor NP onboarding; collaborate with clinical ops and product teams.
• Cover sites within a 30–40-mile radius; mileage and lodging per policy.
