Job summary

Circadia Health is seeking a Nurse Practitioner to join our in-field team across Louisiana. You will see patients first, respond to predictive health signals in real time, and coach facility staff to integrate Circadia into daily care and workflows.

In this full-time role, you will blend direct patient care with technology in bedside integration, training staff, and delivering value through hands-on demonstrations and shadowed assessments.

Qualifications

• Licensed Nurse Practitioner in Louisiana (LA) with ANCC or equivalent.
• 2+ years NP experience with complex adult populations.
• Willingness to travel locally and work in-field across Louisiana.

Responsibilities

• Direct patient care and assessments for flagged residents; update plans of care.
• Train and coach facility staff to integrate Circadia into clinical workflows.
• Interpret trend-based insights to drive timely interventions and reduce readmissions.
• Document notes within AI-assisted tools within 24 hours of encounters.
• Conduct high-volume visits (approx. 25+ per day) focusing on new admissions and Circadia alerts.
• Provide feedback to clinical ops/product teams for workflow improvements.
